I've seen it happen a few times, often from mounting the nozzles too far away from the TB's in the intake tract. Also it becomes a problem if a fuel solenoid sticks open or if a WOT switch that triggers the NOS gets stuck. Direct port is the only safe way to go, not only does it decrease the likelihood of something like this but also provides a much more consistent mixture. Throttle body injection systems on a Z32 pool the fuel to the back cylinders leaving the front cylinders lean and burning valves, etc.
